Association between Multi-morbidity and Health Care Expenditure or Catastrophic Health Expenditures of South Korean Adults

Abstract
The objective of this study was to examine the health care expenditure on an individual level and the household catastrophic health expenditures according to the number of chronic diseases. We used 'the Korea Health Panel', which was a nationally representative longitudinal panel data from 2011 to 2015. The sample was of 16,136 adults (8,164 households), who were aged over 19, as an unbalanced panel study. First, we conducted a panel linear regression to examine the effects of the number of chronic disease on total annual health care costs. Second, panel logistic regression was used to examine the effect of the rate of people with chronic disease on catastrophic health expenditures, and the ROC curve was used to find the cut-off point. Married women aged 50 and over had more medical costs than single women, while catastrophic health expenditures were lower in households who were affected by death. The higher the income, the higher the medical expenditure was, but the probability of catastrophic health expenditures was lower. Compared with medical aid, health insurance subscribers had higher medical expenditures and the probability of catastrophic health expenditures was also high. Lastly, as the number of chronic diseases increased, medical expenses increased. The possibility of catastrophic health expenditures was especially likely to occur from three or more chronic diseases. Therefore, the healthcare policy-makers should be encouraged to reduce not only medical costs for multiple chronic diseases, but also the burden on patients suffering from such diseases.
